    • Issued by health insurance providers.
    • Details who was covered and coverage dates.
  2. Form 1095-C – Employer-Provided Health Insurance
    • Issued by certain employers.
    • Shows coverage offered by employer.
    • Employers offering self-insured coverage issue this form to covered individuals.
  3. Form 1095-A – Health Insurance Marketplace Statement
  • Issued by the Health Insurance Marketplace.
  • Includes details on coverage, policyholders, and enrollment periods.

Important Deadlines:

  • Form 1095-B & Form 1095-C: Due March 31, 2025 from insurers and employers.
  • Form 1095-A: Due February 1, 2025 from the Health Insurance Marketplace.

Do You Need These Forms to File Taxes?

  • Form 1095-B & Form 1095-C are NOT required to file.
  • Form 1095-A IS REQUIRED—if applicable, wait to file until receiving it.

Receiving Multiple Forms

You may receive more than one health care tax form if:

  • You had coverage from multiple providers or employers during the year.
  • You changed coverage or jobs.
  • Different family members had coverage from different providers.

Keep all health care forms with your tax records for reference when filing.
